| Richard
L Lindstrom, M.D., founder and attending surgeon of Minnesota
Eye Consultants and Adjunct Professor Emeritus at the University
of Minnesota Department of Ophthalmology, is a board-certified
ophthalmologist and internationally recognized leader in corneal,
cataract, refractive and laser surgery. He has been at the
forefront of ophthalmology's evolutionary changes throughout
his career, as a recognized researcher, teacher, inventor,
writer, lecturer and highly acclaimed physician and surgeon.

After
graduating from the College of Liberal Arts at the University
of Minnesota, Dr Lindstrom completed his doctorate degree
in medicine in 1972. He conducted research, residency and
fellowship training in cornea at the University of Minnesota
and affiliated hospitals and extended his anterior segment
surgery fellowship training at Mary Shiels Hospital in Dallas
and was a Heed Fellow in Glaucoma at University Hospital
in Salt Lake City.After serving 10 years on the faculty
of the Department of Ophthalmology at the University of
Minnesota, Dr. Lindstrom entered private practice in 1989,
and since then has led the growth and expansion of Minnesota
Eye Consultants serving as managing partner for 15 years.
Dr
Lindstrom holds over 30 patents in ophthalmology and has
developed a number of solutions, intraocular lenses and
instruments that are used in clinical practices globally.
A frequent lecturer throughout the world on cornea, cataract
and refractive surgery, he has presented over 37 named lectures
and keynote speeches before professional societies in the
U.S. and abroad. Dr Lindstrom serves on a number of journal
editorial boards and has published extensively. He is the
recipient of numerous awards for distinguished service by
national and international ophthalmology associations.

| Thomas
W. Samuelson, a founding partner of Minnesota Eye Consultants,
is a board-certified ophthalmologist who specializes in the
medical and surgical treatment of glaucoma and disorders of
the anterior segment, including cataract and refractive surgery.
Dr. Samuelson completed his medical degree at the University
of Minnesota and his residency training at the University
of South Florida where he served as chief resident. He finished
his sub-specialty training at the Wills Eye Hospital at Thomas
Jefferson University in Philadelphia. |

A clinical associate professor of ophthalmology at the University
of Minnesota and past chairman of the medical staff at the
Phillips Eye Institute in Minneapolis, Dr. Samuelson has
played an active role in ophthalmology and in the education
of medical students, residents, fellows, and practicing
physicians
Dr.
Samuelson has contributed research toward the medical and
surgical treatment of glaucoma, especially the management
of coincident cataract and glaucoma. He has also performed
research on the safety of LASIK in patients with other ocular
disorders, especially glaucoma. A frequent lecturer nationally
and internationally on ocular surgery, he has published
numerous scientific manuscripts and textbook chapters on
ophthalmic surgery.

| Dr.
David R. Hardten, a board-certified ophthalmologist and founding
partner of Minnesota Eye Consultants, is one of the nation's
most prominent leaders in the treatment of the cornea, external
disease, anterior segment, cataract, refractive and laser
surgery as well as in research and education.
He
attended the University of Kansas medical school, and was
then accepted in the University of Minnesota Department
of Ophthalmology residency program. |

He
completed his fellowship training in cornea and external
disease at the University of Minnesota and Phillips Eye
Institute.
In addition to an active clinical and surgical practice,
Dr. Hardten has focused on education throughout his career.
As adjunct associate professor of ophthalmology at the University
of Minnesota Department of Ophthalmology, he is actively
involved in teaching residents at Regions Hospital in St.
Paul, as well as fellows at the Veterans Medical Center,
Minneapolis. He is a regular contributor and reviewer for
numerous ophthalmic papers and journals. In addition to
his active practice in medical and surgical diseases of
the cornea and refractive and IOL surgery, Dr. Hardten lectures
throughout the world on refractive surgery and research.

| Elizabeth
A. Davis, M.D., F.A.C.S. is a board certified ophthalmologist
with subspecialty training in corneal, cataract, and refractive
surgery. Dr. Davis completed her doctor of medicine degree
at Johns Hopkins University School of Medicine, graduating
first in her class. For her ophthalmology residency, Dr. Davis
attended Massachusetts Eye and Ear Infirmary, Harvard Medical
School.

Davis is a partner of Minnesota Eye Consultants, P.A. She
is also an adjunct assistant clinical |

at the University of Minnesota and is actively involved
in research. She is an investigator in numerous FDA clinical
studies. She also is an instructor for surgery training
courses. Dr. Davis has given numerous talks across the country
and internationally and has been an invited speaker at many
meetings. She has authored numerous articles and book chapters
in the field of refractive, cataract, and cornea surgery.
Each
year since 2001, Dr. Davis has been one of the top five
LASIK volume surgeons (out of 1,000) within the Laser Vision
Centers, Inc., system in the United States.

J. Riedel , M.D.

Patrick J. Riedel is a board-certified partner of Minnesota
Eye Consultants, PA. He is fellowship trained in the
medical and surgical treatment of glaucoma and cataracts.
In addition, he is specifically trained in multiple
refractive surgery techniques such as LASIK, PRK, and
refractive implant lenses. Dr.
Riedel achieved his medical degree at the University
of Minnesota Medical School. After completing an internship
